Essentially, you start typing into the main field on the top of the page, and it tells you if the .com, .net, or .org is available as you type in every letter. Hard to describe… you just gotta try it. However, if the domain name you ultimately come up with is taken, you can click the “Suggestions” link which will then shoot you over to a page of suggested alternatives for you.
